Johnson City and Elizabethton are connected by US Route 11E, a limited-speed expressway that is part of the Tennessee - Virginia - North Carolina - South Carolina highway system. U-S Route 23 is the main north-south highway in the Blue Ridge Mountains of Tennessee and continues to Roanoke, Virginia; U-S Route 22 in Tennessee and continues to Hickory and Gastonia, North Tennessee; and U.S. Route 23 in North Carolina.

Interstate 81 will take motorists from Knoxville (southwest) to Bristol (northeast) and from there to Nashville (southeast). A Greyhound line, the Johnson City - Elizabethton Line, crosses the city and also serves Johnson County, Tennessee, as well as Bristol and Bristol, North Carolina.
